Atrophic acne scars develop when inflammation damages the deeper skin and healing leaves tissue loss or an uneven structure. We classify the scars before selecting treatment, because their shape determines what needs to be addressed.

• Ice-pick scars: small openings with deep, narrow, often V-shaped depressions. These are a principal indication for TCA CROSS when the individual scar is suitable.
• Narrow boxcar scars: defined edges and a broader base. Some narrow lesions may suit CROSS; width and depth still require examination.
• Rolling scars: broad, wave-like depressions associated with tethering below the skin. Subcision may be needed to release those fibrous bands.

A face can contain several scar types. At BAC, we map scars by subtype, size, depth and skin condition, then match each method to the problem it can address. Using one technique on every scar may leave another structural problem untreated.

An ice-pick opening is often less than 2 mm wide, yet the tract can extend much deeper. A narrow opening alone is not a diagnosis: we also examine the scar edges, base and response to gentle skin stretching. This helps distinguish a focal pit from broader tissue loss or a tethered depression.

TCA creates a controlled chemical injury within the selected scar. A temporary white reaction, known as frosting, can appear as proteins coagulate. The subsequent wound-healing response encourages collagen formation and remodeling. Over repeated healing cycles, the scar wall and base may become less pronounced.

The doctor treats individual pits while limiting injury to surrounding skin. This differs from spreading a peel over the whole face. Precision matters because unintended spread can damage adjacent tissue and may widen or worsen a scar.

Why use a focal technique for a deep, narrow scar?

• Direct targeting: CROSS treats the selected pit rather than relying only on a treatment spread over a larger surface.
• A role in mixed scars: narrow pits can be treated focally while another method addresses tethering or broader texture.
• Flexible treatment extent: isolated scars or suitable scars across the face can be considered, with different pricing options.

The white reaction is not the finished result. Small crusts develop afterward, surface healing takes days, and deeper remodeling continues for months. Immediate swelling may also make a scar look temporarily shallower.

Our doctor chooses the approach according to the scar and your skin. Higher concentration does not automatically give a better result and may increase unwanted effects. Concentrated TCA is for medical treatment and should not be applied at home. The animation explains the mechanism rather than predicting an individual result.

Clinical CROSS protocols use professional-strength TCA, with concentrations in the published literature commonly ranging from 50% to 100%. The concentration is selected with scar anatomy, skin sensitivity and pigment risk in mind. It is one part of the treatment plan, alongside precise application and adequate recovery.

We consider TCA CROSS when ice-pick or narrow boxcar scars are the main concern, the skin is stable, and you can follow aftercare and allow time for visible crusting. Improvement is gradual and several sessions may be needed.

Before treatment, tell our doctor about:

• Active inflammatory acne, infection, open wounds or skin irritation.
• Current or recent isotretinoin use and other acne medicines.
• Keloids, poor wound healing or unusual scarring.
• Dark marks, lighter patches or prolonged pigmentation after procedures.
• Recent peels, lasers, microneedling or other scar treatments.

Active acne or infection may need to be controlled first. A history of poor healing or pigmentation may require preparation, a modified plan or another treatment. Medication timing must be discussed with the doctor; do not stop prescribed treatment yourself.

Darker skin may be considered after assessment, but post-inflammatory hyperpigmentation is an important risk. We review pigment history and explain preparation and sun protection. Skin color alone does not establish suitability or freedom from risk.

The consultation covers medical history, scar classification, skin type, previous procedures and treatment goals. We assess depth and tethering, and may take baseline photographs with your consent to compare progress under similar lighting.

For pigment-prone skin, the doctor may recommend preparation for approximately 2–4 weeks, using an individualized pigment-control or skincare plan. Products and timing are prescribed for your skin; a research protocol is not a prescription to start those medicines yourself.

Depending on the assessment, this may include a pigment-modulating product such as hydroquinone or an adjustment to topical retinoid treatment. These are medical decisions, not a standard combination for everyone. We also review your reaction to previous procedures and whether any area needs to be treated more cautiously.

We explain the area, expected crusting, possible pigment changes, session plan and price before obtaining consent. Tell us if your skin remains irritated after another procedure. The doctor will advise which active products to pause and when to restart them.

Choose a date that allows for visible crusts and sun protection afterward. Focal application can be brief, but another procedure or numbing adds time to the visit. Our team can explain the expected appointment length when you book.

1. Examination and selection: the doctor confirms which pits are appropriate for focal treatment.
2. Preparation: the area is cleansed and prepared, with attention to surrounding skin.
3. Focal application: TCA is placed carefully within selected scars. Brief stinging, warmth and white frosting may occur.
4. Aftercare: we explain cleansing, skincare, sun protection and follow-up before you leave.

The focal treatment typically takes approximately 5–30 minutes, depending on scar number and distribution. This excludes the consultation, preparation and other procedures. General anesthesia is not normally required; comfort measures are discussed for your plan.

A fine applicator is used to keep treatment within the selected scar. The doctor monitors the local reaction and avoids extending treatment onto healthy skin. A visible white point shows the immediate tissue reaction; the subsequent crusting and remodeling still need time.

CROSS may be performed alone or with suitable acne-scar procedures. Combining or staging treatment depends on scar structure, skin condition and the recovery involved.

A typical BAC plan may involve 3–5 sessions. Sessions are generally spaced 4–6 weeks apart, and the interval may be extended to 6–8 weeks, depending on scar depth, skin condition and healing. The treating doctor reassesses the skin before every repeat session.

Waiting for recovery allows the doctor to review crusting, redness, pigmentation and the early response before adding another controlled injury. If the surface is still irritated, healing is delayed or pigment changes need attention, we may allow a longer interval rather than repeat treatment on a fixed date.

Early improvement may become noticeable around 4–6 weeks. A more developed result is assessed over approximately 3–4 months, while remodeling can continue longer. Treatment number and the timing of change vary with scar type, depth and individual healing.

The aim is to reduce the depth and visibility of suitable scars. Complete removal and perfectly smooth skin cannot be guaranteed. Wider boxcar or rolling scars may need another method; some patients need additional sessions after reassessment.

We compare progress with baseline photographs rather than immediate swelling. Scar age, skin type, smoking, sun exposure and aftercare can influence response. Continuing acne control helps prevent new scars.

Remodeling can continue over several months. If improvement plateaus, we reassess the remaining scar structure and the role of other treatments. A wider scar or an area with tethering may need a different method instead of simply increasing CROSS strength or frequency.

Small crusts usually develop in the first 1–2 days. They often remain for about a week and generally separate naturally by approximately days 7–10. Mild redness, tenderness or localized swelling may occur.

Pink or darker marks can remain after crusts have gone. The end of crusting is not always the end of visible recovery. Do not pick; contact BAC for increasing pain, spreading redness, discharge or delayed healing.

Follow-up may include review around days 7–10, then 4–6 weeks and 3–4 months, according to your plan, to assess surface healing, early remodeling and the longer-term response.

• Let crusts fall naturally. Do not pick, scratch, rub or peel treated points.
• Cleanse gently. Avoid scrubs, brushes and exfoliants for at least the first week, and longer if the surface has not healed.
• Moisturize as directed. Use gentle, non-comedogenic products recommended for your skin.
• Protect from sunlight. Use broad-spectrum SPF 30 or higher, often SPF 50, as directed for the recovering surface. Add shade and a hat; limit direct exposure particularly during the first 2–4 weeks.
• Pause irritating actives and heavy makeup. Retinoids, glycolic acid and similar products may need to be paused for 1–2 weeks or until the surface has healed. Follow the doctor’s instructions before restarting.
• Avoid tanning, sauna and steam. We may advise avoiding high-heat exposure for approximately 2–3 weeks, adjusted to the treatment and actual healing.
• Attend follow-up. Report increasing inflammation, pus, a raw or ulcerated point, or crusts that are not healing as expected.

If you also received subcision, INFINI or another procedure, follow the combined instructions. Different treatments may add aftercare requirements. Do not try to correct a healing-related dark mark with another peel.

Mixed scars need a plan for each structural problem. We may use CROSS for narrow pits, subcision for tethered depressions and INFINI RF microneedling for selected broader scars or texture concerns. Picosecond Laser may have a separate role for suitable pigment concerns or other mode-specific goals.

Not everyone needs every procedure. At BAC, the scar map determines the recommendation. We explain what each method is intended to improve and whether to combine treatments or stage them.

Temporary stinging, pinpoint crusts, redness and localized swelling can occur. Possible complications include post-inflammatory hyperpigmentation, lighter pigmentation, delayed healing, infection, persistent redness and widening or worsening of scars.

Darkening is particularly relevant to skin that pigments easily and can last longer than crusting. Published reports also describe worsening when chemical injury extends beyond the intended area. Precise application helps limit avoidable injury but cannot eliminate every risk. [3]

At BAC, we assess the scar first, select the approach, protect surrounding skin and provide aftercare and follow-up. Pigment-prone or sensitive skin may need additional preparation or a staged plan.

Contact us for increasing pain, spreading redness, pus, marked swelling or delayed healing. Persistent color changes also need review. Do not add strong acids, unprescribed whitening products or another procedure while the area is still healing.

Ice-pick scar pilot study: 12 patients entered a study, with 10 evaluated for results. Eight of those 10 were rated as having more than 70% improvement after a treatment series. Temporary lighter and darker pigmentation were each observed in one participant. This small study does not guarantee a BAC patient’s result. [1]

Comparison in 31 patients: different TCA concentrations were used on opposite sides of the face. Both sides improved. Changes in scar scores did not differ significantly between sides, while adverse effects were more serious with the stronger concentration. Treatment should be individualized rather than assuming stronger is better. [2]

Complication report: one published case describes worsening after CROSS delivered with a micropipette. It reinforces the importance of treatment extent and precision but cannot establish how frequently the complication occurs. [3]
